Absorption & Bioavailability

Poor to Moderate — standard commercial tablets have low and variable absorption. A newer enteric-coated formulation (MSI-195) showed roughly 2.8x higher bioavailability than a leading commercial product. Taking it with food delays but does not reduce absorption for newer formulations. Older commercial products may deliver significantly less active compound than the label suggests.

Red Flags to Watch For

  • Standard commercial SAM-e tablets have poor bioavailability — you may be absorbing far less than the label dose
  • Can trigger irritability or mood swings in some users; two patients in a schizophrenia trial experienced worsened irritability
  • Most positive depression studies have methodological flaws — results are promising but not definitive
  • No long-term safety or efficacy data; nearly all studies are 8 weeks or shorter
  • May interact with antidepressants or other psychiatric medications — consult a doctor before combining
  • Not studied or approved for children; the pediatric pain study had only 6 completers and no placebo control
